What is membrane stripping?

The stripping, or detachment of membranes, is a simple technique used to try to naturally induce labor when the term is approaching or has passed.

This medical procedure consists of to pass a gloved finger through the cervix to gently remove the amniotic membrane surrounding the baby from the wall of the uterus.

This movement naturally stimulates the production of prostaglandins, hormones that promote the onset of labor.

This method is performed during a vaginal exam, usually by a midwife or doctor, when certain conditions are met, including that the collar is already a little open (dilated) and the pregnancy has reached at least 39 weeks.    

How does stripping work and how long does it take?

Stripping is a quick gesture, usually performed during a follow-up consultation late in pregnancy.

The procedure therefore lasts barely a few minutes, but it can cause some discomfort or slight pain. It is also possible that spotting (small blood loss) or contractions appear within hours.

Why offer stripping and in what situations?

Stripping is offered in different situations, mainly for avoid classic drug triggering. It is generally indicated  

📅 When the pregnancy exceeds 39 weeks and the cervix is already a little open.

🤰🏻 In the event of prolonged pregnancy (exceeding the term).

🥼 If the medical team wishes to encourage the spontaneous start of labor, particularly if medical factors (hypertension, gestational diabetes) make an upcoming birth desirable.

🐣 Sometimes, at the request of the pregnant woman, to prevent a planned artificial triggering.

In the majority of cases, it is promote physiological birth and to give nature more time, while gently accompanying the body towards birth.

Is stripping effective in inducing childbirth?

According to several scientific studies and recommendations. stripping does not cause certain triggering and systematic, but it actually increases the chances of starting work naturally in the days that follow.

👩🏻‍⚕️ On average, after stripping carried out from 39 weeks, 1 in 8 women (or around 12%) gives birth within 48 hours, and almost 60% give birth within the following week.

🪄 It's not magic, but the intervention has the advantage of being minimally invasive, of limiting the use of synthetic hormones such as oxytocin, and of respecting the physiological rhythm of the body.

If the birth does not occur after one or more strippings, the medical team can then suggest other means of triggering.

What are the risks and side effects of stripping?

Stripping is considered a safe method when performed by a trained professional. However, like any medical procedure, it is not free from possible discomfort or adverse effects.

Here's the bottom line:

 🌹 Pain or discomfort : the maneuver can be unpleasant, even a little painful, especially if the cervix is not very mature.

 🩸 Mild blood loss : a little bleeding after the procedure is common, and not serious. If the quantity is large or signs of bleeding appear, you should consult quickly.

 💪 Irregular contractions : It is common to experience irregular contractions or "false labor" in the following hours or days.

 🦠 Risk of infection : this remains low provided you use a strict aseptic technique. I always ensure the application of barrier gestures if I have to support follow-up during this stage.

💧Premature rupture of the water bag : very rare, this risk requires stopping the maneuver if this occurs.

Stripping generally does not increase the risk of serious complications such as cesarean section.

Can we refuse stripping and are there alternatives?

Yes, of course, consent is always essential! No one is forced to undergo this action, and there are various alternatives. Your choice must be respected, whether you refuse it or wish to postpone it.

Among the gentle alternatives, we find:

 - Acupuncture : known for its action of toning the uterus. Many women in Quebec experience it as very effective support.

 - Sexual relations : they can, thanks to the prostaglandin contained in sperm, help trigger the natural production of these hormones.

 - Nipple stimulation : this technique also promotes the secretion of oxytocin.

- Some mobility and relaxation exercises, guided by a birth attendant.

If stripping does not suit you or if you want to combine several methods, choose the most comfortable and safe approach for you, by talking with your medical team.

How to prepare and what to do after stripping?

To make the experience as peaceful as possible, here are my tips based on supporting many families:

📅 Before: find out, ask all your questions about the progress, the risks and the consequences. If necessary, ask someone to accompany you to the consultation.

📍 The day: eat lightly, dress comfortably and allow time to rest afterwards. Breathe deeply during the exam, this often helps release tension.

💧 After: monitor vaginal discharge, frequency and intensity of contractions||Note any unusual changes (fever, profuse discharge, decreased baby's movements...)

💛 Rest and softness: the day can be emotionally taxing. Give yourself a moment of calm and do not hesitate to ask for help and listening with your doula.
