What is a contraction during childbirth?

Childbirth contractions are the tightening and relaxation of the muscles of the uterus.

They are the "motor" that allows the cervix to open and the baby to progress towards coming into the world. They are often described as a wave or tension that starts in the lower abdomen can extend towards the back and become more and more frequent as childbirth approaches 🤰.  

When we supports families in Quebec, I'm often asked if it's “the right time” or if it's just a false alarm.

Don't worry: this is one of the most universal questions, and everyone feels contractions in their own way, but with a few clues to recognize them!

How to differentiate real contractions from "false labor"?

Among the most common anxieties is the fear of confusing the start of real labor and "false labor" (or Braxton-Hicks contractions). These are irregular, generally painless and do not intensify over time. Conversely, real contractions:

Tip : If you change position, take a bath or drink water and the contraction disappears, it is probably Braxton-Hicks. Otherwise, prepare yourself: the real work perhaps begins !

What are the first symptoms of a labor contraction?

Here are the evocative signs:

  • A feeling of tightness or heaviness in the lower abdomen, which may radiate to the back

  • Pain that gradually intensifies and returns at regular intervals ⏳

  • Hardening of the uterus, sometimes visible to the naked eye or to the touch: the belly forms a “ball”

  • Loss of clear or blood-tinged fluid (called "loss of mucus plug" or rupture of the water bag)

  • A desire to have a bowel movement or a feeling of pelvic pressure

Every woman experiences this moment differently: some speak of it as a very strong period cramp, others feel tension in the kidneys.

How often and for how long do labor contractions last?

A subject that always comes up in preparation for birth: the famous timing of contractions! Those of real work are:

⏰ Regular: at first, they occur every 15 to 20 minutes, but gradually approach (every 5 to 3 minutes during the active phase)

🧘🏻‍♀️ Duration: from 30 seconds at the start up to 60-90 seconds as the birth approaches

🌊 Intensity: increases over time, as does the feeling of impatience or loss of control (which is normal!)

Little tip: keep track of their frequency using from an app or good old pencil paper. This helps to know if it is time to go to the hospital or seek support from your companion.

How to relieve or manage contractions naturally?

Many families want to know how to get through this stage as peacefully as possible, sometimes limiting medical interventions. Here are several methods validated by midwives and doulas.

  • Conscious breathing : learning to focus on exhalation prolongs relaxation and reduces the perception of pain.
  • The movement : walk, swing on a ball (“birth ball”), change position regularly... 🌿
  • Hot baths (or showers): hot water promotes muscle relaxation and offers immediate relief.
  • Massage : Physical contact, especially in the lower back, helps channel pain and reinforce the feeling of being supported.
  • Visualization : images, sweet words and positive anchors, which we often share during emotional preparation for birth.
  • Heat (hot water bottle, heating pad on lower back).

When to induce labor or consult a professional?

Some situations require not waiting:

⏰ Regular contractions every 5 minutes or less, lasting more than ’1 hour (for a first birth)

💧 Loss of amniotic fluid (clear rupture or oozing)

🩸Bright red bleeding

👶🏻 Decreased baby movements

🏥 Unbearable pain that does not subside

👉 If you give birth in a birthing center or hospital, contact your medical team without delay (midwives, obstetrician or companion).

How to prepare mentally and physically for childbirth contractions?

The best preparation? Give yourself the maximum resources, both in terms of knowledge and emotionally 🧘‍♀️. Some ideas:

💪🏻 Participate in a session preparing for childbirth (in person or online)

🧘🏻‍♀️ Working on relaxation: prenatal yoga, mindfulness meditation, sophrology...

✍🏻 Create one birth plan your choices, your desires, your limits, which we can formalize together.

🫂 Surround yourself: life partner, doula or birth attendant, chosen family

💭 Visualize the progress: remember that each contraction brings you closer to your baby ❤️
