When do menstruation return after childbirth? 🕰️

The issue of returning menstruation, also called "returning diapers", is one of the major concerns after childbirth. The answer varies a lot depending on each woman, but there are certain elements that help us see things more clearly.

Generally, menstruation does not return immediately after birth, because the body must first expel lochia, the postpartum bleeding that lasts on average 2 to 6 weeks. Then, the return of menstruation mainly depends on two factors: breastfeeding and hormonal levels.  

Women who choose not to breastfeed generally see their periods return between 5 and 8 weeks after birth.

For those who breastfeed exclusively, the release of prolactin delays ovulation, which can delay the return of periods for up to 6 months, sometimes even longer. That said, there are always exceptions, and each body reacts differently.

How to distinguish lochia from the return of periods?

It is common to confuse lochia and menstruation, both phenomena are indicated by vaginal bleeding. To better understand, here is a comparative table:

Does breastfeeding delay the return of menstruation?

Yes, breastfeeding, particularly exclusive breastfeeding on demand, plays a major role in the time it takes for menstruation to return. It is the lactation hormone prolactin that inhibits ovulation. In some women, this “natural control” of fertility can last for several months.

However, it is important that I emphasize that it is not a foolproof method of contraception. Even without visible signs of menstruation, ovulation can occur before your period returns. There may therefore be a possibility of pregnancy from the first postpartum cycle.

How to prepare for the return of menstruation?

The return of periods after childbirth can be surprising in its intensity, its duration or the associated sensations. I offer you some ideas to better experience this moment:

☁️ Choose gentle hygienic protection (organic cotton towels, ecological menstrual panties, etc.) to avoid irritation of weakened mucous membranes.  

👓 Monitor the regularity of cycles, note changes or anomalies (abundant discharge, pain, fever).

🧘🏻‍♀️ Treat yourself to moments of rest and, if desired, use relaxation or postnatal yoga techniques.

💧 Think about hydration and diet rich in iron to prevent fatigue sometimes felt during heavy periods

Is the menstrual cycle different after pregnancy?

Many women notice changes in their periods after giving birth. Cycles may be longer or shorter, flows heavier or lighter, and some premenstrual symptoms may intensify or improve.

This phenomenon is explained by the hormonal reconfiguration which follows the pregnancy, but also by possible changes in the uterus or cervix, especially if there have been medical interventions (forceps, cesarean section, etc.).

Sometimes, the cycle naturally rebalances after a few months, but if problems persist (very severe pain, abnormally heavy flow, marked irregularity) it is important to talk to a midwife or doctor.

It is essential not to minimize your feelings. Sometimes these symptoms may be related to an infection, uterine abnormalities, or a bleeding disorder. Rest assured: these cases remain rare, but a rapid assessment can prevent complications.

What are some tips for better coping with the return of menstruation after childbirth?

Here are some concrete tips to get through this stage smoothly 👇

  • Anticipate: keep a small protection kit, a hot water bottle and a relaxing herbal tea potion on hand.

  • Communicate with those around you: express your needs for time, space or help, particularly in the face of increased fatigue.

  • Promote the use of natural products for intimate care, such as soothing hydrosols, or suitable essential oils (in agreement with your healthcare professional).

  • Test the practice of a cycle diary: note your emotions, sensations and variations to better understand (and anticipate) future cycles.

  • Treat yourself to one postnatal massage or a support session for reconnecting the perineum.

What contraception is suitable during postpartum?

Fertility returns even before the first menstruation, making contraception an important topic from the first weeks after childbirth. Several solutions exist:

- The breastfeeding method (MAMA method) can work under strict conditions (exclusive breastfeeding, regular interval, absence of return of diapers, infant under 6 months).

- Condoms.

- Certain microdosed pills can be prescribed so as not to disrupt lactation.

- The IUD, hormonal or copper, can be inserted a few weeks after birth, depending on the case.

- Natural methods (symptothermy, observation of signs) require rigorous support; inquire with trained professionals.

How does birth influence the long-term cycle?

Pregnancy, then the arrival of the baby, modify female physiology at various levels: hormonal, emotional, bodily. In the majority of cases, the cycle ends up stabilizing after 6 to 12 months postpartum, but it can also transform in a lasting way (shorter cycles, more regular flow or, on the contrary, more irregular than before).

I also remind you that the years of motherhood, multiplying pregnancies and breastfeeding periods influence the frequency of menstrual cycles and their intensity. Keep in mind that any new situation (stress, fatigue, significant weight loss, etc.) can also cause occasional variations.

If the new rhythm does not suit you or if you feel persistent discomfort, it is strongly recommended to discuss it with a professional, whether a birth attendant, a midwife or your family doctor.
